Study snapshot

This study explores the use of the Cognitive Health Toolkit in the OnTrackNY program. The toolkit includes a manual, tools to help participants with making decisions about cognitive health, and resources for cognitive health.1  

This study asks:
  1. How many OnTrackNY participants received an assessment for cognitive health needs?
  2. What percent of participants experience cognitive difficulties that affect their daily lives or reaching their goals?
  3. What services were participants given for cognitive health concerns?


This study evaluates the use of the Cognitive Health Toolkit at OnTrackNY in the 10 months after teams were trained on cognitive health. Data are based on 933 participants from 18 different OnTrackNY programs.

Study findings

Finding 1. There was variability across OnTrackNY teams in the use of cognitive health assessment tools.

Finding 2. Among participants evaluated, 58% self-reported challenges with their current cognitive ability, while 70% were identified by their provider as a candidate for cognitive health services.

Finding 3. Psychoeducation and skills training were the most common cognitive health services provided by teams when there was a need.

Final thoughts

While there is clear need for the Cognitive Health Toolkit, more studies need to explore the reasons for varying team adoption of, as well as participant satisfaction with, cognitive health services. In this way, services can continue to grow to meet the recovery needs and preferences of participants.
