OnTrackNY is a network of Coordinated Specialty Care (CSC) teams across New York State that supports the well-being of teens and young adults impacted by unexpected changes in their thinking and perceptions. OnTrackNY teams aim to empower participants to make meaning of their experiences and pursue their goals for school, work, and relationships.

OnTrack Central: A collaborative, learning healthcare system dedicated to continual growth

OnTrackNY teams are guided by a centralized organization known as OnTrack Central. As a learning healthcare system, OnTrack Central is dedicated to ensuring that all OnTrackNY teams offer the best, most up-to-date care that science has to offer by maintaining a feedback loop between participants, families, team members, and administrators. 

The OnTrack Central team includes trainers, youth and family members with lived experience, administrators, and more, enabling our work to be informed by a diverse set of perspectives, so we can continuously improve how we support participants. Providing oversight over clinical training and standards across teams statewide, OnTrack Central aims to ensure participants and their families receive the highest quality of care. 

 

Furthermore, as part of the National Institute of Mental Health's (NIMH) learning health care system known as the Early Psychosis Intervention Network (EPINET), OnTrackNY created an initiative called Amplify OnTrackNY. Amplify OnTrackNY engages particpants, families, teams, and others connected to OnTrackNY to learn about ways to improve OnTrackNY and achieve the highest quality of care. Our origin

The New York State Office of Mental Health launched OnTrackNY in 2013 as a result of a large-scale research initiative funded by the National Institute of Mental Health (NIMH): Recovery After an Initial Schizophrenia Episode (RAISE). The RAISE initiative demonstrated that rapid access to specialized, early intervention, in the form of CSC, makes a dramatic difference in young people’s lives and their ability to recover when compared to standard community-based mental health treatment. It also showed the feasibility of offering CSC services in real-word, community settings.

As a result, OnTrackNY was designed to provide specialized intervention services and reduce barriers to rapid engagement in care. Our participants show high rates of participation in treatment, experience a reduction in symptoms, and are more likely to achieve school and career goals. Our goal is to make our services available to every young person in New York who needs them.

Today, OnTrackNY has become a national model for early psychosis care, as one of the largest initiatives of its kind in the country. We offer training & technical assistance to state agencies and mental health providers that strive to implement CSC services nationwide.
