Interactive learning: Learn about Shared Decision Making from OnTrackNY graduates

This interactive learning was created to help you understand Shared Decision Making -- a process that you as an OnTrackNY participant, your support system, and your OnTrackNY team can use to make decisions together. At OnTrackNY, you have the right to make decisions about your services. If you are under 18 years old, you and your parents or guardians can make decisions together with your team. This interactive learning will show you how to build trust with your team.
